Complaints
You may submit your request or issue using the following methods. click on the titles to expand or collapse each section:
Submit A Complaint Using CB14 Request Form (preferred)
Keep in mind the following:
- File a complaint with 311 first: We are often better able to follow up on your request if you can provide a 311 complaint reference number and build a request history within City systems.
- Document your complaint: Keep notes about your concern, using as much detail as possible and record the date/time of each occurrence/incident.
- Collect photo or video evidence: This can help prove that a violation occurred. Do your best to take clear photos and videos. It is also helpful to describe what you are capturing.
1. Click the map below on the location of the problem that you wish to report. A red marker will appear on the map where you clicked and a bubble will pop up above it. You can reposition the marker by clicking on it and dragging it to a new location.
2. Once you have positioned the marker correctly you can click the “Report Here” button to report the problem.
3. Fill out the form with a summary and description of the problem, the address, your name and your email address.
4. You can optionally upload a photo of the problem as well. It is not necessary, but it may be helpful in assessing the problem.
5. Click on “Report Your Issue.”
You can also submit complaints directly from your iPhone, Android phone, or Blackberry.

Keep Track Of Your Complaint
Once you submit the problem we will be notified immediately. You can view and keep track of all the complaints in the area from this page or directly at seeclickfix.com. You will be able to stay up to date on the status of all the problems in the area and will know once they have been resolved.
